“前列腺癌是常见的男性泌尿肿瘤之一，

近年来，我国前列腺癌每年以10%的速度快速增长。

预防前列腺癌牢记早筛


前列腺是男性特有的器官，别看只有栗子般大小，却被称为男人的“生命腺”。


与欧美国家相比，我国前列腺癌患者确诊病程较晚、生存期不理想。


在美国，九成的前列腺癌是通过筛查发现的，


而在我国，前列腺筛查远远不足，导致我国前列腺癌的高致死率。


基于上述严峻状况，邢念增教授指出：”PSA筛查是早期发现前列腺癌唯一有效的手段，应呼吁公众加以重视，早筛早诊。”


前列腺癌筛查主要有两个手段：一是直肠指检。


是指由医生将手指伸进肛门，感受前列腺的大小、质地，有无结节等；


二是前列腺特异抗原(PSA)筛查。


相对来说，直肠指检，痛苦程度和羞耻感较强，公众往往加以避讳。


而PSA筛查只需抽血检查，更加方便，痛苦感更小，患者依从性强。


普及PSA筛查有助于早期发现前列腺癌，尽早治疗，延长生命，提高生活质量。


因此，推荐高危人群定期进行血清PSA筛查，检测结果作为临床监测前列腺癌的重要指标，


能够有效提示前列腺恶性病变情况，为及时治疗提供依据。

高危人群每年一次PSA


针对PSA筛查周期，专家建议以下前列腺癌高危人群，每年进行一次PSA筛查。


一般男性50岁以后前列腺癌高发，应一年做一次PSA筛查；


有前列腺癌家族遗传史，则应从45岁起每年坚持做PSA筛查。


对于PSA筛查结果的解读，专家指出：血清PSA>4.0μg/L是PSA筛查指标的临界值，


如超过这一指标的人群，需进一步检查，明确诊断。


此外，邢念增教授还介绍了预防前列腺癌的方法，除了高危人群一年一次的PSA定期筛查外；


保证健康的生活方式对于前列腺癌预防有积极意义，应注意合理膳食，尽量低脂饮食，多吃蔬菜和水果；


同时，做到不吸烟、不酗酒，注重体育锻炼，避免久坐，保证健康的生活方式。

规范治疗诊疗一体化是关键


对于确诊患者，规范的治疗是其获得长期生存的重要条件，遍布全国300多家PiDT前列腺一体化中心，


通过外科、内科、放疗科、病理科、影像科等多学科的共同协作(MDT多学科综合诊治)，让患者高质量长生存。


一般来说，早期前列腺癌患者治疗预后效果较好。


早期局限性前列腺癌建议做根治手术或放射治疗等方法


，可以选择部分病人延迟治疗，积极观察的方式；


晚期前列腺癌患者，内分泌治疗也是目前临床上比较主流的前列腺癌治疗方案，


可最大程度降低痛苦，改善预后效果，提升5年生存率。


邢念增教授指出：“从预防筛查、诊断、治疗到康复的诊疗一体化对前列腺癌防治很关键。


同时，前列腺癌的治疗还趋向于精准化，为患者制定个体化的治疗方案。”
