尿路结石选体外碎石要慎重


做体外碎石有很多限制，不是所有人有尿路结石都能做，也不是每个医生都会做。


何主任认为，体外碎石最主要的是掌握“碎石能量和碎石频率”，


这对医生来说是最大的难点。尤其是新手医生，能不能打碎结石?


有没有重复高能量碎石?碎石量多堵住了怎么办?


这些都是在操作体外碎石时需要面临的挑战。


如果在操作时连续用高能量机打碎石，那么极易引起肾血肿、肾挫伤、肾绞痛等症状，


后续还有可能出现继发性高血压、糖尿病等问题。


有尿路感染的病人，千万不能马上做体外碎石，得先控制感染。


相比于男性，女性结石患者中尿路感染的会多一些，


因此做体外碎石前更要谨慎，排除危险因素。


另外，当结石病人有妊娠，未纠正的凝血功能障碍，


严重肥胖或骨骼畸形影响结石定位，结石附近有动脉瘤，


结石以下尿路有梗阻等情况时，都是严禁做体外碎石的。

治尿路结石微创方法不少


北京积水潭医院泌尿外科满立波医生强调，事实上，


体外冲击波碎石只适合直径小于两厘米的结石，而且没有尿路梗阻性病变的肾结石患者。


有些医疗机构为了追求经济利益，无选择对体积较大的肾结石多次反复治疗，


不仅没有治愈肾结石，反而造成了严重的肾功能损害。


对于直径大于两厘米的各种肾结石(包括各种肾盏结石和肾盂结石)和肾鹿角型结石，


不适合用体外冲击波碎石，而应采用经皮肾镜手术。


输尿管结石的症状虽然比较重，但处理起来比肾结石容易，方法也更多。


根据结石的大小、部位、梗阻时间和性状可以采用、体外冲击波、输尿管镜等治疗方法。


如果是比较大的膀胱结石，目前通过非开放手术的方法，也能将其打碎取出。


北京积水潭医院泌尿外科，曾用非开放手术的方法，


为一位老先生取出膀胱内置留60年的巨大结石(碎石重100克)。


如果尿路结石的直径小于0.4厘米，90%会自行排出，比如多喝水、多运动，


但如果症状持续或反复发生(肾绞痛、发热、感染、肾积水等)则早到医院治疗。


对于是否需要用药物溶石，泌尿系统大部分的结石都不是尿酸结石，吃药治疗效果并不好。
